You or someone you care about needs help, now what? Get the forms and info you need.

Trying to determine the steps for you or a loved one’s mental health journey can be overwhelming. Below are the next steps to follow, no matter where you are in the journey.

  • Find the Right CG&FS Service or Program

    The first step is to find out which CG&Fs service or program is right for you. We offer counseling and/or psychiatry for young children, school-aged children, teens, adults, and parents/caregivers. Find the right out what service or program would be best for your mental health needs.

  • Request an Appointment

    Once you have determined the right CG&FS service or program, request an appointment. There are three ways to do this:

    1. Call (330) 762-0591 and ask for Admissions, Monday-Friday, 8:30 a.m. – 5 p.m.
    2. Email info@cgfs.org
    3. Fill out an online request form
  • Have an Appointment? Complete the Necessary Admission Form

    Once you have an appointment scheduled, there is a form to complete before you see your therapist. For admission forms for children and adults, there is an online version or a paper version.

    Please note, your completed form must be printed out and brought to the first appointment.
